Job Title: RN Case ManagerJob Description

The RN Case Manager will be an integral part of the PACE program, performing assessments and coordinating interdisciplinary care for participants. You will participate in developing care plans and collaborate with the Utilization Management Committee to ensure comprehensive care delivery.

Responsibilities

• Conduct initial RN assessments for PACE participants.
• Coordinate care for a caseload of PACE participants, ensuring skilled services are delivered as needed.
• Document care delivery, care coordination, and interdisciplinary team (IDT) care planning.
• Develop viable discharge plans in collaboration with patients and their families.
• Support patients throughout the service area, ensuring access to necessary services.
  
  

Essential Skills

• Minimum of 2 years of acute care case management experience, ideally 5+ years.
• Active and unencumbered California RN license.
• Certification in Basic Life Support (BLS) from the American Heart Association.
• Knowledge of case and utilization management practices and methodologies.
• Understanding of state and federal healthcare regulations and accreditation.
  
  

Additional Skills & Qualifications

• Home Health experience is a plus.
• Travel experience is not required.
• Nationally recognized certification in Utilization Management or Case Management preferred.
• Minimum of two years inpatient acute care experience required.
• Minimum of one year of Hospital Case Management or Hospital Utilization Management experience.
  
  

Work Environment

The position is based in the PACE Clinic setting, part of a long-term care model designed to help older adults stay independent and safe in their own homes. This comprehensive care program for seniors with complex medical needs provides services through a coordinated organization, managing everything from primary care to transportation. The program has expanded significantly, serving over 350 participants, emphasizing continual learning and development within a large academic medical center recognized for its excellence and Magnet status.

This is a Contract position based out of Loma Linda, CA.

Pay And Benefits

The pay range for this position is $50.00 - $70.00/hr.
